About Fillable Forms...
First, either "open" or "download" the form of your choice (they are all in Word format). Second, use your keyboard tab to space between information fields, filling in all the blanks (some will be drop down menus which begin as "select." Next, save the completed document to your computer desktop. Then print and sign the forms. You can fax the forms to us at 202-204-8642, or scan the signed documents back to your computer and email them to "info@metropetdc.com," or simply let us know you will be leaving them on the counter for your next MetroPetDC staff visit.
